If you are making a complaint on behalf of another person, we need to check that you are authorised to act on the other person’s behalf. You will need to include evidence with this complaint. The form of evidence you may need depends on the circumstances, but some examples are:
- A copy of an appropriate power of attorney
- A signed letter of authority from the person you are acting on behalf of
- Proof of parental/carer responsibility for a child aged over 12
